This is an example of the Media Library add-on. The players (and the files they represent) are populated from a file set in the Concrete5 file manager. The title and description you are reading are properties you can edit for any file in the file manager.
The add-on provides sort options, so you can sort by title, date, or a custom date attribute you can set yourself for each file, in case the file dates in the file manager don't correspond correctly to your content. Like the Flash Audio and Flash Video add-ons, you can configure options for each of the players. The add-on also limits the number of items per page, and generates paging automatically for you.
